PRE-TRAVEL CONSULTATION

1. In-depth Consultation

Before your appointment you will be asked to fill out the detailed health questionnaire and the patient information pages and to fax them (631-271-3205) to our office.  We will review your specific travel itinerary, your medical history and your current health status and will advise you about the health risks associated with your trip. We will also provide written documentation of that discussion. Recommended and required immunizations will be discussed and provided. Most vaccines can be administered during a single visit, however, occasionally you may need to return at a later date to complete the immunizations series.

POST-TRAVEL

We provide services to those returning traveler with suspected infections.

1. Return Consultation

Consultations are available for the returning traveler who may have contracted an infection or other travel-related medical condition.
