Before Your Office Visit
Be prepared for a visit of at least one hour. Please do not
be late. We rarely keep patients waiting. If
you do arrive late, we may have to reschedule your appointment.
So that we can serve you better, please complete
the following forms before your visit and fax
them (631-271-3205) or bring them with you.
Payment is expected at time of service. We accept
cash, checks, Amex, Visa, MasterCard or debit cards. Medicare
and most health insurance plans do not cover travel vaccinations;
however, you will be provided with a receipt suitable for
submission to your company. Your consultation fee
will depend on the amount of time spent with the doctor.
